  1. STUDENT’S HEALTH RECORD. The state law requires every child entering school to have a current student’s health record (Department of Health Form 14). Parents must submit a completed and current Form 14 at least one week prior to the first day of school. This form is available through your doctor’s office.

  2. TUBERCULIN (TB) TEST and immunizations must be performed within one year of enrollment in our preschool.  The TB test must be administered in the U.S.

  4. TOILET TRAINING. Children enrolled in the 3- to 5-year old program at MCCP are required to be completely toilet trained.  If a child is not toilet trained by the first day of school, enrollment will be deferred and the child will be placed back on the wait list. Our Preschool staff shall assist children whenever occasional accidents occur. The children in the 2-year old program need not be completely toilet trained and can wear pull-up's.  The 2-year old students will complete and master the toilet training process during the course of the enrolled school year.
